To the Editor We are writing to express our strong interest in the study by Dave et al1 at the US Veterans Health Administration nursing home that examines the association between antihypertensive medication use and fracture risk. The study’s findings indicate a substantial increase in fracture risk among participants using these medications. As health care administrators, we recognize the efforts and important societal contributions of this study. However, we also wish to provide constructive suggestions for further enhancement.

First, Dave et al1 did not specify the exact dosage for the initiation of antihypertensive medication. Should even minuscule initial doses of antihypertensive drugs be found to contribute to an increased risk of fractures among older adults, urgent policy revisions would be imperative. To enhance current policy development, we recommend the inclusion of specific details regarding the initial dosages of the medications evaluated.
