What All Physicians Need to Know About the Polio Resurgence in New York State

On July 21 of this year, public health officials a case of paralytic polio in a young adult in Rockland County, New York, northwest of New York City—the first reported instance of polio in the US .

Although was caused by vaccine-derived poliovirus, the patient was unvaccinated and had not traveled to a country where polio is a risk. Genomic sequencing suggests that the virus has been circulating locally under the radar for up to a year, according to the Centers for Disease Control and Prevention (CDC).

Infection with poliovirus, an enterovirus that primarily infects the intestinal tract, is only about 25% of the time, typically causing a flu-like illness. Depending on the virus type, 1% to 5% of people with infections will develop meningitis. About 0.5% to 0.05% of those who are infected—1 in 200 to 1 in 2000 individuals—will develop paralysis after the virus infects the spinal cord, and a small proportion of them will die as a result of breathing problems, according to the CDC.

No Vaccinations are a Failure of Education

The current low rate of vaccinations in much of the US is a failure of education. In high school health classes all vaccine-preventable diseases (diphtheria, polio, measles, etc) with their death rates and other significant sequelae (polio paralysis; measles reduction of immune system capabilities for 2 years, etc) need to be taught, with graphic pictures and, if possible, movies. I wish we could put an iron lung in every high school and make each student stay in it for an hour. Then tell them to imagine being in one for 24 hrs/day for months. They or their parents wouldn't turn down a polio vaccine after that.

Most people don't know how absolutely awful all these diseases are and thus what a miracle and blessing it is that there are vaccines. My mother had pertussis and believe me she made sure we got all our vaccinations. My aunt had polio in the mid-1940's. Charles Durning, the actor, was born in 1923 & had 9 siblings; 5 of them died from smallpox or scarlet fever in childhood, now practically eliminated because of vaccination.
