In Reply We appreciate and fully agree with Dr Loughlin’s comments on our Teachable Moment.1 The normal age-related decline in testosterone levels with aging adds to the challenge of diagnosing hypogonadism in elderly men. Further development of robust age-related reference ranges for testosterone should be established and implemented. Clinical symptoms and signs with appropriate interpretation of the testosterone level is essential when deciding on the need for testosterone replacement.1
