State labor leaders are investigating a worker's death at a youth residential facility.The Iowa OSHA website lists a case open for Ellipsis, Inc., in Johnston, where police say a runaway teenager assaulted Kathleen Galloway-Menke outside the facility last month. Jovahn Mathis, 15-year-old male resident, allegedly shoved Galloway-Menke to the ground, causing her to hit her head, on May 8. She died May 14 after being taken off life support.Mathis has been charged with second-degree murder.Ellipsis's CEO says safety remains the priority. “We are cooperating fully with OSHA and appreciate the work they do with employers to provide safe working conditions.
